Hospital to Home service
If you have a hospital stay due to COPD, you can access Chest Heart & Stroke Scotland’s (CHSS) telephone support service.
Once discharged, you (or a carer) can sign up to receive a series of phone consultations for a period of 6 to 12 weeks.
Calls are tailored to your needs. CHSS healthcare professionals can help with things like:
- self-management of your condition
- emotional support
- financial guidance
